Dr. Borrell developed a special interest in issues of self-esteem and image improvement when he consulted for the Plastic Surgery Department of the Craniofacial Clinic at Baylor College of Medicine with Dr. Melvin Spira. This interest and expertise was further developed by working with clients with eating disorders and weight loss with Dr. John Foreyt at Baylor College of Medicine. Dr. Borrell and his wife, Judy Borrell, founded Positive Changes for Health & Beauty to help individuals have an understanding of their wishes and desires in order to help them achieve their goals of a satisfying and happy life. Other cosmetic surgery and aesthetic surgery clinics usually have one physician who supervises and provides the services. Positive Changes for Health & Beauty recruits physicians with expertise and abilities to provide a variety of services based on the varying needs of different clients, as well as the ability to work as a team of individuals who are interested in enhancing the beauty potential of each individual. Dr. Borrell has been an innovator in providing and developing medical and surgical services. He founded several hospitals and clinics locally and nationally. He was president and chairman of the board of Comprehensive Weight Management. He is actively involved in Interactive Health Systems (see masteringstress.com). He founded National MedServ with Dr. J. Martin Barrash, a company that provides MRI services in joint venture with physicians, and non-invasive treatment for kidney stones. He founded M.D. Weight Care in 1995 in consultation with John Foreyt and other national experts on weight loss. he has been a consultant to Medeva Corporation and Wyeth-Ayerst for pharmacological treatment of obesity. In 1999, after discussions with his college roommate, Dr. Gaspar Lazzara, Chairman of the Board of Orthodontic Centers of America, he opened Positive Changes for Health & Beauty. His goal was to bring together experienced professionals in aesthetics and cosmetic surgery who could provide comprehensive beauty enhancement services and self-image improvement at an affordable price.

In the year 2000, he founded the Cosmetic Surgery & Aesthetic Center of America in order to develop clinics in joint ventures with physicians and spas. Dr. Borrell is active in the Houston community and a member of the University Club and the City Club. He is a founding stake holder in the University of Houston Center of Entreprenuership and Innovation, and a member of the steering committee of the Houston Technology Center.
